Student Forms
Here are the common forms that you may need. Please download the form and follow the instructions.
Student Enrollment Agreement :
If you are not using web registration for courses, use this form to enroll in your courses. Print the form, complete it, sign it and fax, email or mail it to the Registrar.
Transcript Request Form:
If you need an official transcript, use this form. Print the form, complete it, sign it and fax, email or mail it to the Registrar.
Add/Drop Form:
If you want to add or drop a course in which you are already enrolled, use this form. Print the form, complete it, sign it and fax, email or mail it to the Registrar.
Program Petition:
If you want to change your degree program, add a degree program or change from a Special Status student to a student earning a degree, use this form.
Petition for Graduation:
If you are planning to complete your degree during Fall, Spring, or Summer in the 2010-11 academic year, complete this form. Print the form, complete it, sign it and fax, email or mail it to the Registrar.
Multi-Purpose Academic Petition:
If you need to petition for a Course Continuance, an extension in a course, or a request to take an exam/assignment early or late, use this form. Print the form, complete it, sign it and fax, email or mail it to the Registrar.
Leave of Absence / Withdrawal:
If you need a leave of absence for up to one year, complete this form to secure your place at the seminary and your governing catalog. If you need to completely withdraw from the Seminary, complete this form. Print the form, complete it, sign it and fax, email or mail it to the Registrar.
